Sample inspector narratives
During the inspection I observed two infants ages 13 and 15 months in the main room with children as old as 6 years old.
Infant group was observed to be in the outdoor playground at the same time as toddler and pre-k group. The children were observed playing in the same area.
During an investigation it was found that the operation had 14 children in care in one room where the youngest was 4 months old and the oldest was 5 years old. There was more then an 18 month gap between the two children.
