Sample inspector narratives
Operation did not account for all children when loading and unloading children off the van. This caused a child to be left on the van for approximately 30 minutes.
There was not a formal way to account for children exiting the vehicle on 5-18-26.
During transportation, the bus driver nor the school age teacher accounted for the children as they exited the van This was corrected during the inspection when I observed the second transportation drop-off and the children were accounted for.
